This Agreement is for a 12-month membership, beginning on the date you submit this .
Member agrees to remain active for the full contract term unless terminated according to this Agreement.
CANCELLATION POLICY
Member agrees to the full 12-month term.
Early cancellation may occur only for:
Medical disability (documentation required)
Relocation (over ___ miles)
Other approved hardship
Early termination fee: $100 OR remaining balance.
LIABILITY WAIVER
Member understands fitness activities involve risk.
Member releases and holds harmless Gym/Company, owners, trainers, and staff from injury, loss, or damages resulting from participation.
Member confirms they are physically able to exercise.
