MEDIA RELEASE: I, the undersigned parent/guardian, grant permission for my child to be photographed and/or recorded during participation in the RUE Arts Academy Summer Intensive. I understand that these images and videos may be used for promotional purposes including social media, website content, and marketing materials.
LIABILITY WAIVER I, the undersigned parent/guardian, understand that participation in dance and physical activity involves some risk of injury. I agree to release and hold harmless Reaching Unified Enlightenment (RUE) Arts Academy, its instructors, staff, and affiliates from any and all liability for injuries or damages that may occur during participation in this program. I confirm that my child is physically able to participate in dance activities.
PAYMENT AGREEMENT I understand that tuition for the RUE Arts Academy 3-Week Summer Intensive is $200 per week, totaling $600 for the full program. 2 I acknowledge the Early Bird Special rate of $135 per week is only valid if payment is made in full by May 1st. I agree to the following:
