A doctorally prepared nurse practitioner, board certified in adult gerontology acute care, is present for up to four hours. She visits your loved one herself and observes what a record cannot show. Mental status, comfort, work of breathing, skin, positioning, and every line and tube. She reviews what is available in the record, and then finds out directly from the nurse and the physicians on the unit what has actually happened and what was actually done, which is frequently different from what is planned.
She attends the care conference. She asks the questions a family does not know to ask, as a clinician speaking to clinicians rather than as a relative hoping to be heard. She checks the equipment with her own eyes. She escalates whatever she finds to the person who can act on it.
And you receive a written clinical summary the same evening, organized by system, with what is still outstanding and who owns each item.
WHAT IS INCLUDED
Up to four hours on site. Bedside clinical observation. Review of the available record. Direct conversations with nursing and with the physicians on the unit. Care conference attendance where it falls that day. Equipment and safety check. Escalation of anything found. A written clinical summary the same evening.
Travel within sixty miles is included and does not come out of your four hours.
Beyond four hours, a further day rate applies, and never without your approval first.

Why four hours?
Because four hours covers a care conference, the bedside, the record, and the conversations with every service on the unit without being rushed. Most days need less. The ones that need more are usually a full week rather than a longer day.
Does travel come out of my time?
No. Travel within sixty miles is included and the four hours begin when she arrives.
Can I buy more than one day?
Yes. Days are purchased individually and no day is ever added without your written approval, so the total stays in your control. Families who need several usually find the seven day Hospital Advocacy Week is better value.
What if the date changes?
Hospitals move. Tell us and we move with it at no charge.
Can this be the day of a transfer?
Yes, and it is one of the best uses of a single day. Transfer day is when the discharge medication list is compared against what the receiving facility actually writes, and when pending results get assigned to a named person. That hour is worth more than most full days.
Do you need to see the record first?
Not necessarily. For a straightforward day, no. For a complicated picture, the Comprehensive Clinical Review at $2,500 is the better starting point, because arriving already knowing the admission is worth considerably more than arriving cold.
