You get direct access to a doctorally prepared nurse practitioner and board certified patient advocate who directs and coordinates your loved one’s care, without you having to manage it alone.

What’s included:

• A named clinician directing and coordinating care

• 24 hour phone access

• Remote advocacy with the care team, specialists, and facility leadership

• Guidance through care transitions and discharge planning

• Written updates for the family

• 50% off bedside visits and all episodic and a la carte services during your engagement

Investment: $3,950 per month, three month minimum, prepaid ($11,850)

This engagement is remote access only and does not include bedside visits. Hi Society Health is not the treating provider. All sales are final once the agreement is signed and payment is made.
