Most families find us during a crisis. What they discover afterward is that the crisis was not the problem. The problem is that nobody is holding the whole picture between crises, and so the next one arrives the same way the last one did.
Your mother has a cardiologist, a nephrologist, a primary care physician and a specialist she sees twice a year. Each is deciding on a picture that is missing whatever the others changed last month. The medication list she carries is not the one in any of their systems. Nobody is tracking whether the test ordered in March was ever done.
This is the arrangement where that stops.
A doctorally prepared nurse practitioner, board certified in adult gerontology acute care, holds the whole picture continuously. Not during an admission. Always.
WHAT HAPPENS EVERY MONTH
One visit in the home, with a written summary afterward organised by system.
One appointment attended, with the specialist briefed in writing beforehand so the twelve minutes in that room are spent on decisions rather than on history.
A clinical contact with you every two weeks.
Medication reconciled after every prescriber change, not quarterly. When the cardiologist adjusts something on Tuesday, the nephrologist knows on Tuesday.
One current reconciled record of your parent, maintained continuously and carried to every encounter. Most families have never held that document, because it has never existed.
Ordered tests tracked to result, so the test ordered is the test performed, the result is received, and somebody acts on it.
AND WHEN SOMETHING HAPPENS
Because it will. The difference is that the clinician who arrives already knows the history, the medications, the baseline, and what your parent would say about any of it.
Admission services are purchased separately and activate immediately, with no intake delay, because the picture is already current.
WHO BUYS THIS
Families who have been through one admission and do not want to meet the next one unprepared.
Anyone managing several specialists who do not speak to one another.
Families who found us for a crisis and realised the crisis was the symptom.

FAQ
How is this different from buying a week when something happens?
A week is a response. This is the arrangement that means the week is rarely needed, and that when it is, the clinician arrives already knowing everything rather than starting from nothing.
Is there a version for someone out of the area?
Yes. Distance Family at $1,950 a month is built for the adult child who lives elsewhere. One home visit, a weekly written report, a scheduled call each month, and all provider communication handled here.
Is there a version with more?
Yes. Concierge at $3,950 a month is two home visits, two appointments attended, a weekly clinical contact, twenty four hour access, and half fee on every admission service. Beyond that is a retained engagement, which begins at $60,000 annually, has no meter at all, and includes unlimited inpatient direction with no admission fees. Retainers are quoted after a clinical review.
Can I add more visits in a month?
Yes, at the on site day or remote session rate, with your approval first.
Can I stop?
Yes, on thirty days notice after the three month minimum, and you keep the complete reconciled record.
