This engagement covers two components.
Comprehensive Clinical Review. A doctorally prepared nurse practitioner, board certified in adult gerontology acute care, reviews the complete available medical record system by system. Neurologic status, heart and rhythm, breathing and oxygen, digestion and bowels, kidneys and fluid balance, blood and infection, mobility and skin. Every medication is reconciled against the pre-admission list, including the question that is rarely asked, which is what should now be discontinued. Any adverse placement determination is reviewed against the grounds stated for it. Every pending result is recorded with the service responsible for it. You receive a written care direction plan before the on site day begins.
The published fee for this review is $2,500. It is included here at $1,250.
Extended On Site Day. Up to eight hours at the bedside and within the facility, plus the written clinical summary the same evening.
Bedside clinical observation of mental status, comfort, work of breathing, skin, positioning and every line and tube. The discharge medication list compared line by line against the pre-admission list. Every pending result assigned to a named person. Diet, swallowing, wound care, code status and any advance directive confirmed as recorded and travelling with the patient. Equipment and specialty surfaces confirmed as arranged rather than ordered.
Where a transfer is involved, written confirmation is obtained from the receiving facility that it can deliver the care actually required, and a clinician to clinician handoff call is made before arrival.
For the duration of the day, the clinician is the point of contact with both facilities. The family is not required to call anyone.
Independence
This practice is engaged to establish what is clinically true and to secure a safe transition. It is independent of the facility and independent of the family. It takes no part in any complaint, grievance, regulatory filing, legal action or media contact, and its documentation is not produced for those purposes. Findings are the clinician’s own and are documented as found.
Optional further direction
Additional days may be elected separately and are not included in this fee. On site, up to four hours, $1,250. On site, up to eight hours, $2,500. Remote session, two hours, $625, with each further two hour increment at $625 and never worked without written approval first.
What this is not
This is not home care, companion care, or nursing care. Hi Society Health, Inc. is not the treating provider. It does not diagnose, prescribe, order diagnostic studies, admit, discharge, or deliver treatment, and it performs no physical examination. It is not an emergency service. In an emergency, call 911. Services are private pay and are not billed to any insurer. No outcome is guaranteed, including any clinical outcome, any placement, or any determination by a payor.
