General Liability Release and Informed Consent Agreement

Effective immediately upon payment, I, the client, hereby acknowledge and agree to the following terms and conditions regarding my engagement with Baylee Falk Coaching L.L.C. (the "Company") for health and wellness services. By submitting payment for services, I confirm my understanding and acceptance of this agreement:

Release of Liability:

  1. Scope of Release:
    By making payment, I fully and forever release and discharge Baylee Falk Coaching L.L.C., including its agents, employees, affiliates, successors, and assigns, from any and all claims, injuries, demands, damages, actions, or causes of action of any kind or nature, whether known or unknown, arising out of or related to:

    • Fitness Coaching

    • Nutrition Coaching

    • Hormone Health Coaching and Testing

    • Mindset and Lifestyle Coaching

    • Yoga Instruction

    • Any other health and wellness services provided by the Company

  2. Acknowledgment of Risk:
    I understand and acknowledge that participation in health and wellness programs involves inherent risks, including but not limited to physical injury, health complications, or other unforeseen outcomes. I voluntarily assume all risks associated with these services.

  3. Non-Medical Disclaimer:

    • I understand that Baylee Falk is not a licensed medical doctor or healthcare provider.

    • If hormone testing is ordered, I acknowledge that these tests are for informational and educational purposes only and are not intended to diagnose, treat, cure, or prevent any medical condition.

    • All recommendations provided by Baylee Falk Coaching L.L.C. are based on fitness, nutrition, and lifestyle changes aimed at improving overall health and well-being. These recommendations do not constitute medical advice.

    • I am advised to consult with a licensed healthcare provider for any medical concerns or conditions.

Terms of Agreement:

  1. Payment and Services:
    Payment for services rendered constitutes acknowledgment and acceptance of this agreement. Payment is considered complete and final, and no refunds will be issued unless mutually agreed upon in writing.

  2. Entire Agreement:
    This Release represents the full and final agreement between the parties. Any modifications must be made in writing and agreed upon by both parties.

  3. Governing Law:
    This Release shall be governed by and construed in accordance with the laws of the State of Wisconsin.

Acknowledgment:

By making payment for services, I acknowledge that I have read and understand this agreement in its entirety. I accept and agree to its terms, and I voluntarily release Baylee Falk Coaching L.L.C. from any liability as described above.
