SUNROSE WELLNESS (ADRIANA LICERIO)
GENERAL WAIVER & RELEASE OF LIABILITY
This Waiver & Release of Liability (“Agreement”) applies to all services, sessions, programs, events, workshops, immersions, group offerings, corporate offerings, digital content, and experiences (“Services”) provided by Sunrose Wellness (Adriana Licerio).
By purchasing, booking, or participating in any Sunrose Wellness Services, I acknowledge and agree to the following:
1. VOLUNTARY PARTICIPATION & ASSUMPTION OF RISK
I understand that my participation in Sunrose Wellness Services is entirely voluntary. I knowingly and willingly assume all risks associated with participation, whether known or unknown, foreseeable or unforeseeable.
These risks may include, but are not limited to: physical injury, emotional or psychological distress, intense emotional release, pain, illness, dizziness, fainting, shortness of breath, paralysis, disability (temporary or permanent), economic or emotional loss, or death. I understand these risks may occur during or after participation, including while traveling to or from any session or event.
I acknowledge that I am participating entirely at my own risk.
2. NOT MEDICAL, MENTAL HEALTH, OR FINANCIAL ADVICE
I understand and agree that Sunrose Wellness Services are not a substitute for medical, psychological, psychiatric, mental health, or financial diagnosis, treatment, or advice.
Sunrose Wellness (Adriana Licerio) is not acting as a licensed medical provider, psychologist, psychiatrist, therapist, or healthcare professional. No guarantees or medical or financial outcomes are promised.
I agree that I am solely responsible for my physical, mental, emotional, and financial well-being before, during, and after participation.
3. PARTICIPANT RESPONSIBILITY & DISCLOSURE
I affirm that I have truthfully disclosed any physical, medical, psychological, emotional, or mental health conditions that may affect my participation.
I understand that it is my responsibility to consult with a qualified healthcare provider if I have any concerns regarding my participation. I acknowledge that failure to disclose relevant conditions is solely my responsibility.
I understand that I may stop, slow down, modify, or discontinue my participation at any time and that I am responsible for honoring my own limits.
4. CONTRAINDICATIONS & INTENSITY OF SERVICES
I understand that certain Sunrose Wellness Services, including breathwork, hypnosis, EFT (tapping), NEE (Neural Energetic Encoding), embodiment practices, and energy work, may result in intense physical, emotional, or psychological experiences.
Common contraindications may include, but are not limited to:
- Cardiovascular conditions or heart disease
- High or abnormal blood pressure
- History of aneurysms
- Epilepsy or seizure disorders
- Severe psychiatric conditions (including psychosis or paranoia)
- Bipolar disorder
- Osteoporosis
- Recent surgery or injury
- Glaucoma or retinal detachment
- Pregnancy
- Active addiction
- Use of heavy or major medications
- Any mental illness without adequate treatment or support
I acknowledge that I have consulted, or have had the opportunity to consult, with a qualified healthcare provider prior to participation if needed.
5. RELEASE OF LIABILITY
To the fullest extent permitted by law, I hereby release, waive, discharge, and forever hold harmless Sunrose Wellness (Adriana Licerio), including its owners, facilitators, employees, contractors, volunteers, partners, agents, successors, assigns, affiliates, and venue owners, from any and all claims, demands, actions, causes of action, damages, or liabilities of any kind, including those arising from negligence, related to my participation in any Services.
6. INDEMNIFICATION
I agree to indemnify, defend, and hold harmless Sunrose Wellness (Adriana Licerio) from any and all claims, damages, losses, costs, or expenses (including attorney’s fees) arising from my participation or from any claim brought by me or on my behalf.
7. MEDICAL CARE & FINANCIAL RESPONSIBILITY
In the event that I require medical care or treatment, I agree that I am financially responsible for all costs incurred. I understand that I should carry my own health insurance.
8. MEDIA RELEASE
I grant permission for Sunrose Wellness (Adriana Licerio) to use photographs, video recordings, audio recordings, or other media taken during my participation for marketing, educational, or promotional purposes. I understand this content may be shared publicly, including on social media and digital platforms.
9. COMMUNICABLE DISEASE ACKNOWLEDGMENT
I understand that participation in in-person Services may expose me to communicable diseases, including but not limited to COVID-19. I voluntarily assume this risk and release Sunrose Wellness (Adriana Licerio) from any liability related to such exposure.
10. SEVERABILITY
If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.
11. GOVERNING LAW
This Agreement shall be governed by and interpreted in accordance with the laws of the State of California.
12. ACKNOWLEDGMENT & CONSENT
I affirm that:
- I am 18 years of age or older
- I have carefully read and fully understand this Agreement
- I understand that this is a release of liability and a legal contract
- I am signing freely and voluntarily without coercion
By purchasing, booking, or participating in Sunrose Wellness Services, I provide my full consent to this General Waiver & Release of Liability.
