🌉 As you step into The Bridge™, what are you most ready to have support with in this season? Check all that apply.
🌉 Membership & Recurring Billing Agreement I understand that The Bridge Membership™ Plus+ is a recurring monthly membership that includes two (2) private sessions per billing month. My payment will automatically renew each month until I cancel my membership.
📅 Session Use & Rollover Agreement | I understand that my membership includes two (2) sessions per billing month. Sessions must be used during the applicable membership period and unused sessions do not roll over, accumulate, transfer, or convert to credit for a future month.
🔄 Rescheduling & 24-Hour Policy | I understand that appointment changes must be made at least 24 hours before my scheduled session. Changes requested within 24 hours of my appointment may result in the session being forfeited.
⏰ Late Arrival & Missed Session Policy | I understand that my session begins and ends at the scheduled time. Arriving late will reduce my available session time and will not extend the appointment. Missed appointments/no-shows are considered used sessions and cannot be rescheduled, credited, or refunded.
💳 Payment & Refund Policy | I understand that membership payments are non-refundable once processed, including if I choose not to attend or use all of my included sessions during that billing period.
🚪 Membership Cancellation | I understand that I may cancel my membership before my next scheduled renewal to prevent future charges. Canceling does not provide a refund for a payment that has already been processed, and my membership benefits end according to the terms of my current billing period.
💬 Communication & Access | I understand that The Bridge Membership™ Plus+ provides ongoing support through my two scheduled private sessions each month, but does not include unlimited messaging, texting, on-demand calls, or unscheduled access between appointments. Additional sessions or services may require separate booking and payment.
✨ Nature & Scope of Services | I understand that sessions may incorporate spiritual/intuitive guidance, astrology, tarot, coaching, strategy, and personal development depending on my needs. These services are intended for guidance, education, reflection, and personal development and are not a substitute for medical, mental health, legal, financial, or other licensed professional services.
🔒 Final Membership Acknowledgment | I confirm that I have reviewed and understand the expectations, recurring billing, scheduling, rescheduling, cancellation, session-use, communication, and refund policies associated with The Bridge Membership™ Plus+ and agree to these terms as a condition of joining.
