Participants should consult their primary care provider before making any changes to their healthcare routine.
10.1.3 1:1 Follow-Up Visits: Participants are entitled to one-on-one follow-up visits every two (2) weeks, commencing from the date of purchase, for the duration of the 90-day program. Failure to book sessions in a timely manner will result in forfeiture of those visits without refund or rescheduling options (subject to Section 8.1).
10.1.4 Laboratory Evaluations (if applicable): If your program includes laboratory evaluations, it is your responsibility to complete the required lab work in a timely manner once the lab requisition has been released to you. There will be no refunds for labs that are not completed. Additionally, lab tests included in the program cannot be substituted with alternative tests outside of what the program offers. Laboratory evaluations provided as part of this program are for informational purposes only and should not be used as a sole determinant of medical diagnosis or treatment. No substitutions are permitted, and failure to complete the assigned lab work does not warrant a refund.
10.1.5 Participant Responsibility: This program is designed to facilitate a comprehensive approach to health within a condensed timeframe. Participants acknowledge that achieving optimal results requires active engagement, commitment, and adherence to the program’s structure and responsibilities.
10.1.6 No Guarantees & Medical Disclaimer: Participation in The Colby Method does not guarantee specific health outcomes. This program does not replace professional medical care, diagnosis, or treatment. Participants should consult a qualified healthcare provider before making any medical decisions or changes to their health regimen. These are provided for educational and informational purposes only and are not medical advice. Participants acknowledge potential risks, including but not limited to allergic reactions, interactions with existing medications or conditions, nutritional deficiencies, physical injury from exercise, or adverse effects from supplements or peptides (such as injection-site reactions, hormonal imbalances, or gastrointestinal issues). The Company does not guarantee the safety or suitability of any recommendation for any individual. By participating, you release and waive any claims against the Company, its representatives, or affiliates for any health deterioration, injury, or adverse outcomes resulting from your implementation of program recommendations, even if the Company was advised of the possibility of such risks. You agree to consult a qualified healthcare provider to assess personal risks before following any guidance.
10.1.8 Limitation of Liability: The Colby Collective LLC, its representatives, and affiliates shall not be held liable for any adverse health outcomes, injuries, or damages arising from the implementation of this program. Participants assume full responsibility for their health and well-being and agree to consult with their healthcare provider for any concerns.
10.1.9 No Guarantee of Results: Results from this program may vary based on individual health factors, adherence, and lifestyle. The Colby Collective LLC makes no guarantees regarding specific health improvements, weight loss, hormone balance, or symptom relief.
10.1.10 Arbitration Clause: Any disputes arising from participation in The Colby Method shall be resolved through binding arbitration in Idaho. By enrolling in this program, participants waive their right to a trial by jury or participation in a class-action lawsuit.
By enrolling in this course, participants agree to these terms and acknowledge their responsibility for engagement, scheduling, and timely completion of any applicable lab work.

Failure to schedule visits in a timely manner will result in forfeiture of those visits without the option for rescheduling or refund (subject to Section 8.1).
10.2.4 Laboratory Evaluations (if applicable): If your program includes laboratory evaluations, you are responsible for completing the required lab work once the lab requisition has been provided. There will be no refunds for labs that are not completed. The lab tests included in the program cannot be substituted with alternative tests outside of what is offered. Laboratory evaluations are intended for informational purposes only and should not be used as the sole determinant of medical diagnosis or treatment. Participants will have lifetime access to wholesale pricing for supplements, as long as the provider continues to have access to supplements at wholesale cost. It is the participant's responsibility to purchase these supplements directly from the designated source.
10.2.6 Peptide Therapy (if applicable): If your program includes peptide therapy, you acknowledge and agree to the following terms as part of your informed consent for peptide therapy:
You voluntarily elect to pursue peptide therapy with the goal of supporting health optimization and addressing dysfunction associated with your specific condition(s). You understand that results are not guaranteed, and outcomes vary based on individual factors.
You acknowledge that peptide therapy may be considered experimental, controversial, or unnecessary by the mainstream medical community and may not comply with conventional medical standards. Many peptides are not approved by the Food and Drug Administration (FDA) for your intended use, and their use may be off-label or investigational.
The Colby Collective will provide guidance and monitor your therapy to minimize potential side effects, but cannot guarantee the absence of adverse reactions, injuries, or other health risks. You accept all risks associated with peptide therapy.
You are solely responsible for self-administering the prescribed peptide(s) in accordance with the dosages, methods, and instructions provided by The Colby Collective. Failure to adhere to these guidelines may increase risks and is at your own liability.
You understand that peptide therapy does not replace other treatments and must disclose all treatments, including peptides, to your healthcare providers to avoid potential interactions or complications.
Alternative options for your condition(s) include, but are not limited to, lifestyle modifications (e.g., diet, exercise), surgical interventions, FDA-approved medications, or other therapies, which you are encouraged to discuss with a licensed healthcare provider.
You acknowledge that peptides are not intended to diagnose, treat, cure, or prevent any disease or medical condition, as defined by the FDA, and no such claims are made by The Colby Collective.
You understand that peptide therapy costs are unlikely to be covered by insurance, and you are responsible for all out-of-pocket expenses. A $300 credit will be applied toward the purchase of peptides included in the program. Any additional peptides beyond this credit must be purchased at your own cost.
You acknowledge that peptide availability, legality, or sourcing may be subject to change due to regulatory actions or market conditions, and The Colby Collective is not liable for disruptions in therapy due to such changes.
By enrolling in a program that includes peptide therapy, you confirm that you have been fully informed of and understand the potential risks, benefits, and alternatives to peptide therapy. You provide your informed consent to participate, acknowledge that The Colby Collective operates in a coaching capacity and not as a licensed medical provider, and agree to hold The Colby Collective, its representatives, and affiliates harmless from any adverse outcomes related to peptide therapy.

By participating, you release and waive any claims against the Company, its representatives, or affiliates for any health deterioration, injury, or adverse outcomes resulting from your implementation of program recommendations, even if the Company was advised of the possibility of such risks.
