Emotional Freedom Techniques (EFT) may be used as a supportive tool for emotional, mental, or physical well-being. However, you - the client - remain fully responsible for your use of EFT during and after sessions. Celeste Loetz is not a licensed medical or mental health provider. She offers EFT Tapping as a trained and certified practitioner through the Gala Darling Method. While EFT has shown positive and sometimes remarkable results, it is still considered an emerging modality and should be viewed as a complement - not a substitute - for professional medical or psychological care. Please consult your physician, licensed therapist, or qualified health care provider regarding any questions or concerns about your physical or mental health. Compliance with all prescribed medical treatment, therapies, and medications is expected. By submitting this form, you acknowledge that you assume full responsibility for your personal health and safety during and after EFT sessions, and you release Celeste Loetz and Turnt Up Tapping from all liability related to your participation.
Consent for EFT Session
I authorize Celeste Loetz to use EFT as part of our work together.
I understand that:
• Celeste Loetz does not diagnose, prescribe, or treat any medical or mental health conditions.
• EFT sessions are not psychotherapy or medical treatment; they are a complementary, wellness-based approach focused on emotional regulation and personal growth. • I agree to inform Celeste of any significant medical or mental health concerns that may be relevant to our work
• I take full responsibility for my emotional and mental well-being during and after sessions. If I have any questions or require additional support, I understand that Celeste is available for reasonable follow-up by email or text
Session Recording & Transcription
I understand that I may be offered the option to have the audio and/or video of my sessions recorded. Audio or video recording is optional, and I may decline or withdraw my permission without affecting my ability to receive services. I understand and agree that, whether or not I consent to an audio or video recording, Celeste Loetz may create and retain a written transcript of my sessions for professional quality review, accurate documentation, and the preparation of personalized tapping scripts, session summaries, or notes for me. Transcription technology may process the session audio for the purpose of producing the written transcript; however, Celeste Loetz will not retain an audio or video recording unless I have separately consented to its retention. Any transcript, recording, session summary, tapping script, or related documentation will be treated as confidential and stored securely. Secure technology service providers may process or store this information solely for the purpose of providing transcription or data-storage services and are expected to protect its confidentiality and not use it for unrelated purposes. This information will not otherwise be disclosed or used for training, educational, promotional, or other external purposes without my express written consent, except when disclosure is required by law.I may request a copy of any recording, transcript, tapping script, or session summary retained by Celeste Loetz at any time.
